I provide the Charity Fund "Happy Child" with the necessary documents and data: photos, bank account details and give my permission to publish this information and photos on the Internet and in the media.

To place a request, I provide copies of documents.

ОI undertake to report monthly on donations received in various ways.

I undertake to spend the incoming donations only on the elimination of this problem.

If the funds collected with the information support of the fund and the www.deti.zp.ua portal were not used for their intended purpose, I undertake to transfer them to the Fund to help other needy families.
